Term: Usual; Term: Customary; Term: and Reasonable Fees
The maximum amount of money that a health plan will agree to pay for a specific medical service or procedure. Most commonly referred to as Prevailing Charges. Also referred to as Usual & Customary Reimbursement or Reasonable and Customary. (See also Prevailing Charges).
