Term: Hospice Care Program

A coordinated, interdisciplinary program that provides services that consist of inpatient and outpatient care, home care, nursing care, counseling, and other supportive services and supplies provided to meet the physical, psychological, spiritual, and social needs of the dying Employee or Dependent; and drugs and medicines (requiring a Physician's prescription) and other supplies prescribed for the dying Employee or Dependent by any Physician who is a part of the Hospice Care Team; and instructions for care of the patient, counseling, and other supportive services for the family of the dying Employee or Dependent.
