| Term: Home Health Care |

| A program of home care that: is required as a result of a sickness or injury; and follows a period of Hospital confinement; and is a result of the sickness or injury that was the cause of the Hospital confinement; and is established in writing by the attending Physician within seven days after Hospital confinement ends; and is certified by the attending Physician as a replacement for Hospital confinement that would otherwise be necessary. One Home Health Care visit will be counted for up to four hours of service (in a 24-hour period) by a Home Health Aide and one visit will be counted for each visit by any other person.
