Why can’t I get information on my loved one’s benefits?

The Health Insurance Portability and Accountability Act of 1996 (HIPAA) dictates that health plans like the CBEBT cannot legally share an adult member’s protected health information (PHI) with any outside party, unless we have written permission to do so. If your loved one would like you to have access to his or her PHI, please click here to obtain a printable authorization form, which can be sent to us by mail or fax.
