Health Benefits and Claims

What is the difference between my deductible and my co-pay? Does my co-pay apply to my deductible?

Your deductible is the amount you are responsible for before the Plan begins to pay benefits for eligible charges. The deductible will apply to all charges except those where you have a co-pay.

Your co-pay is the amount you are required to pay a provider at the time of service. This co-pay does not apply to your deductible. Your co-pay will apply to your out-of-pocket.

Note: Members enrolled in the Religious Medical Trust please refer to your Trust Benefit Booklet for your benefits or contact customer service toll-free at 800-807-0500.
