PPO Providers
How do I verify that my health care provider is a member of my PPO?
The name of your PPO and the provider verification telephone number are listed on page 1 of your Summary of Benefits and Coverage. You can find your Summary of Benefits and Coverage by logging into your Participant account at mycbs.org/health and then click on My Coverage Summary. The click on the "Click to Display" Link by Medical Plan Summary, then click on the "SBC_###_#_###" link. You PPO will also be displayed directly under your Name after you log into your Participant account at mycbs.org/health. If you click on the My PPO link it will display instructions on how to find a provider as well as a link that takes you directly to the provider search area.
