| Do I need to call for Pre-Certification for Hospitalization? |

| If you are scheduling a hospitalization, you must call for pre-certification. Please call the toll-free number on the back of your medical ID card. In an emergency situation, you or a family member must call within two working days of admission. If you do not call for pre-certification or if you fail to call after an emergency admittance, your benefits will be reduced. |
