| Are routine or well benefits paid by the Plan? |
|
| Routine benefits have always been paid by your Plan. In addition, the Plan provides an annual limited preventive benefit paid at 100% for PPO providers. After this limited benefit has been paid, routine services are still covered at normal Plan benefits. Please visit Your Employee Benefits booklet and refer to the Medical Summary of Benefits in the Medical section for more information. The routine benefits covered at 100% by your Plan are described under the heading Preventive Care Benefit - PPO Providers Only.
