How can I get a replacement member ID card?
You can easily print your ID card online. Log into your account at mycbs.org/health and click on the View Your ID Card link to print. If you need replacement or additional cards, call our customer care department at 1-800-807-0400 for Employee Benefit Trust members or 1-800-807-0500 for Religious Medical Trust members or email customer care at hbscustomerservice@cbservices.org .
