| How do I find out what is covered under the plan? |

| First refer to your Summary of Benefits and Coverage. After logging into your Participant Account, click on My Coverage Summary. Click on "Click to Display" link by the Medical Plan Summary heading which will display a quick plan overview. Click on the "SCB_###_#_###" to view your Summary of Benefits and Coverage.
You may also click the "Plan Book" link to view more details of your Summary Plan Document.
If there is a particular service you are wondering about and cannot find it in the above referenced documents, our customer service department will be able to assist you. Contact a representative at the number listed on your ID card.
